Dall’11 Agos­to 2020, for­ti di più di tre lus­tri di espe­rien­za di coun­selling interna­zionale, a grande richi­es­ta, inau­guri­amo un nuo­vo servizio: il con­sul­to online. Preci­siamo, però, fin da subito, che esso – che avver­rà in totale pri­va­cy e diret­ta­mente e uni­ca­mente con il prof. Guc­cia­r­do – non sos­ti­tu­isce una visi­ta diret­ta (cui si pre­sup­pone ci si sia già sot­to­posti negli ulti­mi 20 giorni con uno spe­cial­ista di fiducia).

A par­tir du 11-08-2020, après plus de 15 ans d’ex­péri­ence inter­na­tionale, à la demande générale, nous inau­gurons le ser­vice des con­sul­ta­tions en ligne. Cepen­dant nous pré­cisons tôt que ces con­sul­ta­tions — qui auront lieu directe­ment et unique­ment avec M. Guc­cia­r­do, et en toute intim­ité — ne rem­pla­cent pas une vis­ite directe. On sup­pose que vous avez déjà été exam­iné, au cours des 20 derniers jours, par un spé­cial­iste de votre confiance.

From 11th August 2020, after more than 15 years of inter­na­tion­al coun­sel­ing expe­ri­ence, by pop­u­lar demand, we inau­gu­rate a new ser­vice: the online con­sul­ta­tions. We spec­i­fy, how­ev­er, imme­di­ate­ly, that these con­sul­ta­tions — which will take place direct­ly and only with Gian­lu­ca Guc­cia­r­do, MD, in full pri­va­cy — does not replace a direct vis­it. It is assumed that you were already exam­ined, in the last 20 days, by a trust­ed specialist.

Il servizio online consente di ricevere un consulto, un controllo del pia­no terapeutico, la prescrizione di esami di laboratorio o strumentali inte­grativi etc.
Si può chiedere aiuto per problemi:
  • nella gestione della voce artistica;
  • nella gestione acrobatica durante performance canoro-coreutico-musicale;
  • di voce, linguaggio, deglutizione o comunicazione;
  • di area bioetico-sessuologica.

Si può optare per:
- CONSULTO VIA EMAIL
- CONSULTO VIA VOIP (SKYPE o WHATSAPP o ZOOM)

Come procedere
Si compili il form (vedi: sotto) affinché da parte nostra si possa valutare se il ser­vizio è idoneo al vostro caso. In un uni­co file .pdf (non si potrà accettare alcun altro formato), si alleghi ogni informazione anamnestica, ogni relazione di visita re­cente e ogni esame ematochimico e di radiodiagnostica che si ritenga essere inerente al problema per il quale bisogna ci si spieghi in poche righe perché ci si chiede di aiutarvi. Inviando l’email, si dichiara contestualmente di essere maggiorenni, di ac­cettare le Normative inerenti alla Privacy e al trattamento dei dati personali e altresì di sapere e accettare che, avvenendo le consultazioni attraverso Gmail e/o Skype e/o Whatsapp e/o Zoom etc. (tutti servizi erogati da Aziende con Sede fuori dall’Europa) i propri dati – nonostante l’impegno del dott. Gucciardo e delle Aziende stesse (come leggesi nelle loro Carte dei Servizi) a non diffonderli – potrebbero essere visibili ai (e stoccabili dai) suddetti gestori non europei di servizi.
Se vi si risponderà che possiamo aiutarvi, si effettui il pagamento (nei modi facili che si troveranno ben spiegati nella nostra email) e se ne invii la ricevuta all’indirizzo voice.md.it@gmail.com.
In tempi rapidi (e comunque entro 72 ore), vi si invieranno:
– modulo (da firmare) del consenso informato al trattamento dei dati personali
– ricevuta sanitaria che può essere usata ai fini fiscali
– per chi ha optato per il consulto via Skype o Whatsapp o Zoom, due proposte di data e ora­rio per esso
✅ Sia il consulto via email sia quello via VOIP avverranno entro 15 giorni lavorativi
SI NOTI BENE: via email o via VOIP non è possibile fare diagnosi né accer­tare la veridici­tà delle informazioni che a noi siano state spedite. Il nostro con­sulto (che si fonda sulla to­tale fiducia sia in chi scrive sia nell’operato dei colleghi che hanno potuto effettuare la vi­sita reale) funge, quindi, da sola erogazione di un suggerimento clinico-diagnostico-terapeutico che avviene come completamento e non in so­stituzione di una visita medica reale e diretta. Se non è stata effettuata, fin da ora vi invitamo a sottoporvi a essa piuttosto che a un (nostro) parere specialistico online.
INFORMAZIONE IMPORTANTE. A pagamento avvenuto, in caso di annullamento del consulto da parte del paziente, il 25% della quota versata non sarà rimborsabile. Invece, se l’annullamento avviene da parte nostra, il 100% della quota sarà rimborsato all’interessato.

La façon de procéder
Remplissez le formulaire ci-dessous, afin que nous puissions évaluer si le service vous convient. Écrivez deux lignes pour nous dire pourquoi vous écrivez pour obtenir de l’aide, et joignez toutes vos informations sur vos antécédents médicaux et chaque rapport de visite récent et chaque examen sanguin et / ou radiodiagnostique que vous pensez être inhérent au problème pour lequel vous demandez de l’aide. Tous les documents doivent être remplis dans un seul fichier .pdf (nous ne pouvons accepter aucun autre format). En envoyant cet e‑mail, vous déclarez simultanément que vous avez 18 ans ou plus, et que acceptez la Réglementation relative à la confidentialité et au traitement des données personnelles. Vous déclarez aussi que vous savez et acceptez que, ayant des consultations via Gmail et / ou Skype et / ou Whatsapp et / ou Zoom etc. (tous les services fournis par des Sociétés basées hors d’Europe), vos données — malgré l’engagement du Dr Gucciardo et des Sociétés elles-mêmes (en tant que l’on peut lire dans leurs cartes de services) pour ne pas les diffuser — pourraient être visibles à (et stockables auprès) les fournisseurs de services (non européens) susmentionnés.

Notre consultation (qui repose sur la confiance totale dans le travail des collègues qui vous ont rendu visite visuellement auparavant, et en vous-même) est la fourniture d’une suggestion clinique-diagnostic-thérapeutique qui se déroule comme un complément et non en substitution d’un réel examen médical direct auquel, s’il n’a pas été réalisé, vous êtes maintenant invité à vous soumettre à la place que à notre avis en ligne.

How to proceed
Fill in the form below, so that we can assess whether the service is suitable for you. Write two lines to say us why you’re writing for help, and attach every your medical history info and every recent visit report and every blood and/or radiodiagnostic exam that you believe to be inherent to the problem for which You ask for we help you. All documents have to be filled in a single .pdf file (we can not accept any other format). By sending this email, you simultaneously declare to be 18+, and to accept the Regulations relating to Privacy and to the processing of personal data, and to know and to accept that, having consultations through Gmail and / or Skype and /or Whatsapp and / or Zoom etc. (all services provided by Companies based outside Europe), your data – despite the commitment of Dr. Gucciardo and of the Companies themselves (as you can read in their Cards of Services) not to disseminate them – could be visible to (and stockable by) the aforementioned non-European service providers.

PLEASE NOTE: by email or via VOIP it is not possible to make a diagnosis or to ascertain the veracity of the information that has been sent to us. Our consultation (which is based on the total trust in the work of colleagues who have visited you visually before, and in yourself) is the provision of a clinical-diagnostic-therapeutic suggestion that takes place as a completion and not in substitution of a real and direct medical examination. Please, if it has not been carried out by you, submit yourself to it, rather than to our online opinion.
